Comparison of Transdermal Diclofenac Patch with Oral Diclofenac as an Analgesic Modality Following Removal of Mandibular Impacted Third Molars: A Cross over Efficacy Trial

Abstract
Background: This study attempts to compare two different drug delivery systems in the management of postoperative pain following surgical removal of impacted mandibular third molars using diclofenac sodium as a standard drug which was administered in twodifferent routes, oral and transdermal.

Methods: Forty subjects between the age group of 18?40 years with bilateral mesioangular impacted mandibular third molar teeth, who underwent surgical removal under local anaesthesia. Four subjective pain scales were used to record the postoperative pain withreadings taken at 2 hours, 4 hours, 8 hours, 12 hours and 24 hours on the 1st, 2nd and 3rd postoperative day. Following removal of the impacted mandibular third molar on one side the patients were given oral Diclofenac (Voveran SR) 100 mg once daily. 2 weeks later removal was done on the contralateral side followed by administration of Diclofenac sodium transdermal patch (Nu Patch) 100 mg once daily.

Results: Transdermal diclofenac sodium had a more effective pain relief compared to oral diclofenac sodium on the first postoperative day with a P value of 0.01. However, there was no statistical or clinical difference on the second and third postoperative days.

Conclusions: Results of this study indicate that the transdermal diclofenac patch provides a more potent analgesia as compared to the oral diclofenac tablets. Thus it may be used for routine post extraction analgesia in those patients who are susceptible to gastritis and with poor patient compliance.
